Agreement of Waiver and Release of Liability
Waiver. I voluntarily agree to participate in Awaken.Yoga classes. I recognize that this work involves physical activity and movement, which may carry some natural risks. I am responsible for my own experience and my own safety. I also understand that I am responsible for continuing to monitor my physical limits. I have no medical conditions that would endanger me in any way or prevent my participation in the classes. I waive any right to take action against Awaken.Yoga, LLC for any reason.
No Guaranty. Awaken.Yoga, LLC has made every effort to accurately represent the program and its potential benefits. Results can and do vary, therefore Awaken.Yoga, LLC makes no guarantees. The testimonials and examples used are not intended to represent or guarantee that anyone will achieve the same or similar results. Each individual’s success depends on many factors, including but not limited to, his or her background, dedication, starting point, desire and motivation.
Disclaimer. This program does not provide medical, chiropractic, psychological, or psychiatric advice and the information provided to participant is not intended as such. Participant should refer all inquiries related to medical, chiropractic, psychological, or psychiatric advice to appropriately qualified professionals.
Indemnification. Participant shall indemnify, defend and save harmless Awaken.Yoga, LLC, its officers, agents and employees from and against any and all loss, cost (including attorneys' fees), damage, expense and liability (including statutory liability and liability under workers' compensation laws) in connection with claims, judgments, damages, penalties, fines, liabilities, losses, suits, administrative proceedings, arising out of any act or neglect by Awaken.Yoga, its
agents, employees, contractors, participants, invitees, representatives, in, on or about the operation of the program. This indemnity shall survive the termination of this Agreement.
Participant hereby releases Awaken.Yoga, LLC from any and all
liability or responsibility to participant or anyone claiming through or under participant by way of subrogation or otherwise for any injury, loss or damage of sustained by participant covered by any insurance then in force.
Release. I, my heirs, and legal representative forever release, waive, discharge and covenant not to sue Awaken.Yoga, LLC, its agents or any associates for any claim I may have for injury, loss, or damages that I may sustain as a result of my participation in Awaken.Yoga, LLC classes.
I have read the waiver and release of liability and fully understand its consent. I voluntarily agree to the terms and conditions stated above.
